Study: Case management after discharge reduces mortality in heart failure patients

A Cochrane Review found that using a case management approach, where patients are monitored by telephone calls and home visits, usually by a nurse, reduced mortality one-year after discharge compared to more traditional methods. The researchers examined 25 trials of 5,942 people. Read more.
