Walgreens partners with hospitals to provide pharmacists to coordinate medications for patients

Walgreens’ new WellTransitions program uses pharmacists to help coordinate patients’ medications during admission to and discharge from a hospital. Pharmacists would go to the hospital when patients are admitted and deliver medications they need when they are discharged. About 12 hospitals are currently participating in the program, which aims to reduce hospital readmission. Read more.
